Smart Pharmacy Prescription Information

Smart Pharmacy is a well-established Florida based Pharmacy that opened in April of 2007. We specialize in the art and science of compounding a variety of custom-made medications by physician request. The unique service of compounding allows specialty formulations specifically made for each patient that would not be available commercially at general retail pharmacies.

Smart Pharmacy accepts most insurance plans. Cash Option Formulas are available for all patients in the event that the prescribed medications are not covered by insurance.

How to fill your prescription(s):

1. Your Physician can fax your prescription directly to Smart Pharmacy
2. You can bring the prescription from your Physician into Smart Pharmacy located at 3740 St Johns Bluff Rd Suite 19 Jacksonville, FL 32224 during normal business hours.

How to refill your prescription(s):

With SMART Refills, you don’t have to lift a finger. We will always contact you to let you know when your medication is ready to be refilled according to your dosage schedule. Once your medication has been filled, we will keep you updated on when it’s ready to be picked up, delivered or shipped.

How to transfer your prescription(s):

It’s easy to transfer a prescription from another pharmacy to Smart Pharmacy. In fact, it’s as easy as calling Smart Pharmacy at 877-811-6337(MEDS) and dial “0” to speak with one of our Customer Care Representatives. You will only need to provide a Smart Pharmacy representative with your name, phone number, current pharmacy and current medications.
Smart Pharmacy will contact your current pharmacy and begin filling your prescription. If the prescription has no remaining refills, we will contact the prescribing physician.
